Parent Alternative Pick Up Form 2025-2026

This form allows parents to designate alternative persons to pick up their student(s) when they cannot, such as for doctor or dentist appointments. 

Alternate persons must be 18 and come to the office and present their ID.  When a person has not been authorized by this form to pick up a student, the office must call a parent/guardian for approval to release the student.  If approval cannot be verified with the parent/guardian, the student will not be released.  

The Parent Alternative Pick Up Form 2025_2026 must be completed and submitted to the office each school year.
