Course Selection Forms 2026-2027
6th grade Course Selection Directions:
- Enter the student’s first and last name.
- Select the elementary school the student currently attends.
- Enter Parent/Guardian 1 name and email.
- Enter Parent/Guardian 2 name and email (if applicable).
- Indicate whether you have completed online registration for the 2026–27 school year.
- Select any support services the student currently receives.
- Rank the four electives (Art, PE, 21st Century Learning, Ceramics) from 1–4, with 1 being the top choice.
- Rank the music options (Band, Choir, Orchestra) in order of preference.
- Check the box if the student already owns an instrument they plan to use.
- Add any questions or comments (optional).

- Enter the student ID number.
- Indicate whether online registration has been completed.
- Select at least one yearlong elective (Chorus, Band, Jazz Band, Orchestra, Piano, Spanish 1, or PE).
- For each yearlong class listed, select Yes or No.
- Rank semester electives (Art, Leadership, PE, STEM, Wellness) from 1–5, each with its own number.
- Indicate interest in Yearbook or Peer Tutor (applications required).
- Enter any questions or comments (optional).

- Select at least one yearlong elective (Advanced Chorus, Band, Jazz Band, Orchestra, Piano, Spanish 2, or PE).
- For each yearlong class listed, select Yes or No; some require previous experience.
- Rank semester electives (Art, Leadership, PE, STEM, Wellness) from 1–4, each with its own number.
- Indicate interest in Yearbook, Aide, or Peer Tutor (applications or approvals required).
- Provide names of teachers who have pre‑approved aiding (if applicable).
- Enter any questions or comments (optional).
